  1. Copy of your current license/certification from your state
  2. Copy of your Errors & Omissions Insurance
  3. Resume with 3 references
  4. W-9 form
  5. Sample appraisal (1004 full appraisal – preferably an FHA appraisal or complex property assignment)
  6. Indicate date you began appraising real estate or date first licensed
  7. State why you would like to join our network
